Important Changes for Pear Tree Medical 

Dr. Jeffress is joining Memorial Hermann Medical Group -  Central Pearland   

I hope this note finds you happy and healthy. I am writing to let you know of a change in our healthcare relationship. After much thought and consideration, I am closing Pear Tree Medical Associates after 11 years and will be joining Memorial Hermann Medical Group (MHMG) in central Pearland. MHMG in Central Pearland (3203 E. Broadway) is about 3 miles away from my current location. Below are some important dates you should note: 

• Last Day Seeing Patients at Pear Tree Medical: December 15, 2021 
• First Day Seeing Patients at MHMG: January 3, 2022 

I am so thankful for the many patients and their families who have trusted their care to me over the years, and I look forward to the opportunity to continue to care for you at my new practice location. It is very important to me that you maintain continuous healthcare. If you continue under my care, please complete the medical records release form. You can return the signed form in email to records@peartreemedical.com .

• For appointments on or after January 3rd, you can call the Memorial Hermann Medical Group office now at 281-485-8876. The office staff will be happy to assist you in scheduling an appointment.

If you will be transferring care to another provider, please complete the medical record release form indicating where you want your records sent and return it by email to records@peartreemedical.com.
